Alarming levels of drug-resistant TB found worldwide

LONDON (Reuters) - Scientists have found an alarming number of cases of the lung disease tuberculosis in Africa, Asia, Europe and Latin America that are resistant to up to four powerful antibiotic drugs.

In a large international study published in the Lancet medical journal on Thursday, researchers found rates of both multi drug-resistant TB (MDR-TB) and extensively drug-resistant TB (XDR-TB) were higher than previously thought and were threatening global efforts to curb the spread of the disease.

"Most international recommendations for TB control have been developed for MDR-TB prevalence of up to around 5 percent. Yet now we face prevalence up to 10 times higher in some places, where almost half of the patients ... are transmitting MDR strains," Sven Hoffner of the Swedish Institute for Communicable Disease Control, said in a commentary on the study.

TB is already a worldwide pandemic that infected 8.8 million people and killed 1.4 million in 2010.

Drug-resistant TB is more difficult and costly than normal TB to treat, and is more often fatal.

MDR-TB is resistant to at least two first-line drugs ? isoniazid and rifampicin - while XDR-TB is resistant to those two drugs as well as a powerful antibiotic type called a fluoroquinolone and a second-line injectable antibiotic.

Treating even normal TB is a long process, with patients needing to take a cocktail of powerful antibiotics for six months. Many patients fail to complete their treatment correctly, a factor which has fuelled a rise in the drug-resistant forms.

Researchers who studied rates of the disease in Estonia, Latvia, Peru, the Philippines, Russia, South Africa, South Korea and Thailand found almost 44 percent of cases of MDR TB were also resistant to at least one second-line drug.

Tom Evans, chief scientific officer at Aeras, a non-profit group working to develop new TB vaccines, told Reuters treatment options for XDR-TB patients were "limited, expensive and toxic".

Treatments for drug-resistant TB can cost 200 times more than those for normal TB, he said in an emailed statement. They can also cause severe side effects like deafness and psychosis, and can take two years to complete, he added.

In the United States, MDR-TB treatment can cost $250,000 or more per patient, and in many poorer countries costs can be catastrophic to health systems and patients' families.

"Without a robust pipeline of new drugs to stay one step ahead, it will be nearly impossible to treat our way out of this epidemic," Evans said.

SPREADS THROUGH AIR

Tracy Dalton from the United States Centers for Disease Control and Prevention, who led the Lancet study, said that so far, XDR-TB has been reported in 77 countries worldwide.

"As more individuals are diagnosed with, and treated for, drug-resistant TB, more resistance to second-line drugs is expected to emerge," she said.

The spread of these drug-resistant strains was "particularly worrisome" in areas with poor healthcare resources and limited access to effective drugs, she added.

TB is a bacterial infection that destroys patients' lung tissue, making them cough and sneeze and spread germs through the air. Experts say anyone with active TB can easily infect another 10 to 15 people a year.

The World Health Organization (WHO) predicts more than 2 million people will contract MDR TB by 2015.

A report by non-governmental organizations in March said a $1.7 billion shortfall in funds to fight TB over the next five years meant 3.4 million patients would go untreated and gains made against the disease will be reversed.

In their research, Dalton and colleagues found rates of resistance varied widely between countries.

Overall, resistance to any second-line drug was detected in nearly 44 percent of patients, ranging from 33 percent in Thailand to 62 percent in Latvia.

In about a fifth of cases, they found resistance to at least one second-line injectable drug. This ranged from 2 percent in the Philippines to 47 percent in Latvia.

XDR-TB was found in 6.7 percent of patients overall. Rates in South Korea, at 15.2 percent, and Russia at 11.3 percent, were more than twice the WHO's global estimate of 5.4 percent at that time.

Robin Hemley is the author of ten books of nonfiction and fiction and the winner of many awards including a 2008 Guggenheim Fellowship, The Nelson Algren Award for Fiction from The Chicago Tribune, The Story Magazine Humor Prize, an Independent Press Book Award, two Pushcart Prizes and many others.? His fiction, nonfiction, and poetry has been published in the U.S., Great Britain, Germany, Japan, the Philippines, and Hong Kong, and he teaches creative writing workshops around the world.? He has been widely anthologized and has published his work in such places as The New York Times, The Believer, The Huffington Post, Orion, The Wall Street Journal, The Chicago Tribune, New York Magazine, and literary magazines.? The BBC is currently developing a feature film based on his book Invented Eden that tells the story of a purported anthropological hoax in the Philippines. His third collection of short stories, Reply All, is forthcoming in 2012 from Indiana University Press (Break Away Books) and The University of Georgia Press recently published his book A Field Guide For Immersion Writing: Memoir, Journalism, And Travel (reviewed by Steve Weinberg in this month?s What?s in Store column). He is a senior editor of The Iowa Review as well as the editor of a popular online journal, Defunct (Defunctmag.com) that features short essays on everything that?s had its day.? He currently directs the Nonfiction Writing Program at The University of Iowa and is the founder and organizer of NonfictioNow, a biennal conference that will convene in November 2012 in Melbourne, Australia.? ?

Your CV is intimidating. Have you always been a writer?

I?ve always written, but I haven?t always identified myself as a writer.? I come from a literary family: My mother was a short story writer, translator, and novelist, and my father was a publisher, novelist, poet, and translator. So, from an early age, part of my life included writing and reading.?

Your focus, for the most part, has been short stories, essays, and memoir.??What is it about these genres that grabs you?

It?s true I like the short form?essays and stories?though I?ve gone long several times, as well.? My novel, The Last Studebaker, was well-received and reviewed, and I?m only now working on a second novel.? I love the novel form, too, but I tend to write in whatever form interests me at the time.?

??????????? Workshops tend to focus on the short story. When I went to the Iowa Writers? Workshop as a young writer, I wrote a lot of stories, some of which made it into my first book of stories. I loved short stories well before that.? I used to love to peruse my family library and pull short story collections from the shelves.? That?s how I discovered Kafka, Borges, and Isaac Babel, among others.

??????????? After graduate school, for a couple of years I taught part-time at The School of the Art Institute of Chicago, and there I taught ?Essay Writing.?? This was a basic composition course, but here, I fell in love with the essay, too, largely through an anthology that the poet Donald Hall edited.? At the time, hardly anyone in the academy thought of the essay as an art form that held its own against the short story and the poem.? That?s all changed now, and I?m glad of it.? I think the short form interests me so much because these forms tend to focus on what?s not written as much as what is written.? The gaps, what?s left out, what?s left to the reader?s imagination?this is what makes these forms so vibrant to me.??

??????????? Ha, sorry, my silly sense of humor displays itself.? Okay, crossover.? With my family background, you can see that crossover is inevitable.? I?ve never been one to declare genre loyalty.? Sure, writers tend to excel at one form, but there are many writers who don?t want to limit their creativity to just one form, and that?s certainly how I feel about my own writing.???

Tell me about your new book, A Field Guide for Immersion Writing.

??????????? This book arose directly from my book, Do-Over, in which I revisited past embarrassments and failures from my childhood.? When I was asked what I?d call this type of writing, I said it was an immersion memoir.? The term ?immersion? has long been applied to journalism but never before to memoir?when I was speaking to an editor at The University of Georgia Press about this type of writing, she suggested I write a book on it.? Over time, I expanded the idea to consider immersion writing in all its forms: memoir, journalism, and finally, travel writing?which of course involves immersion as well.?

This book crosses over a little bit with another book of yours, Turning Life

into Fiction, which has sold more than 50,000 copies.? I love life-based fiction. ?What prompted you to write it?

Turning Life into Fiction, likewise, came out of my engagement with the subject and a discussion with an editor, Lois Rosenthal, who founded Story Press and briefly revived Story Magazine.? I had been teaching about the transformation process in fiction, the ways in which we turn experience into fiction, and Lois asked me if I?d write a book about this for her.? I believe it was the first book that Story Press published.? Later, when Lois and her husband Richard sold their publishing enterprise, the book somewhat inexplicably went out of print, though it was selling well.? And so, Graywolf Press picked it up and it?s now in its second or third printing with them.??

Both A Field Guide and Turning Life into Fiction talk about use what

you're living through and write about it.??Is this the main way you find your

subject matter?

I find my subject matter in a lot of different ways.? As a writer, I?m fairly eclectic.? I?ve written from dreams and I?ve written about people who have nothing to do with what I?m living. One story in my forthcoming collection of short stories is from the point of view of a Portuguese spy on the ship of the explorer Magellan and another takes place outside of Chino, France, in the time of Joan of Arc.? Ideas present themselves in many forms, and if they intrigue me enough, I take them on.

Do you believe immersion journalism is a more...valid brand of journalism?

I wouldn?t use the term ?valid,? but I do believe that traditional journalists can be blind to their own agendas.? I?ve experienced this personally?how a self-righteous journalist can completely muck up a story and still remain self-righteous.?

??????????? My experience with this comes from my book, Invented Eden: The Elusive, Disputed History of the Tasaday.? This told the story of a group of 26 people who were ?discovered? living in the rainforest in supposed complete isolation in the Southern Philippines in 1971.?

??????????? For a while they were hailed as the ethnographic find of the century and a 45,000 acre reserve was given to them by the Marcos Administration.? For a while they were a world-wide sensation, supposedly living an existence that closely resembled the cave-dwelling lifestyles of our Pleistocene-era ancestors.

??????????? ?Then in 1986, a freelance reporter from Switzerland hiked into the rainforest unannounced and was told through interpreters that the Tasaday were a hoax, that they were simply local farmers who had been coerced into playing Caveman.?

??????????? Good story, but it wasn?t true.? The guy hiked in and out of the rainforest in a couple of days and believed his translator, a local anti-Marcos activist who had an agenda of his own.? Now, the world declared that they weren?t the find of the century, but the hoax of the century, and credulous reporters followed, all filing supposedly objective stories that agreed the Tasaday were a hoax, without knowing anything about the complex political and cultural situation of the Philippines at the time.

??????????? It?s called ?confirmation bias,? the idea that you pay attention to arguments and ?facts? that support your theories, and ignore or throw out anything that disagrees with your theories.? Everyone is susceptible to it, even supposedly objective journalists.? In this case, they created a mess.?

??????????? I spent five years researching the story, traveling around the world and meeting everyone alive who was a part of the story: journalists, academics, even the Tasaday themselves, and I found out that the story was a lot more complex and that the real hoax was perpetrated by the people who said the Tasaday were a hoax.? Yet the conventional wisdom today is still that the Tasaday were a hoax, mostly because of bad reporting that was broadcast on ABC?s 20/20 and taken as gospel.?

??????????? I simply think it?s good to own up to your biases, to allow the reader to see that a human being, and not a supposedly infallible news organization, has written the story and gathered the information.?

When doing immersion journalism or travel writing, I assume it's best to

come across as much like a non-writer as possible?

I wouldn't say that?s always the case; it?s only true of writers who write what I call an ?infiltration.?? Ted Conover, for instance in New Jack, when he was writing about being a guard at Sing Sing.? Or Kevin Roose when he, a liberal student from Brown, infiltrated the conservative Christian Liberty University in Lynchburg, Virginia.? Short of that kind of infiltration, I think it?s good to be honest about being a writer?and I think that there are ethical considerations involved in the infiltration as well.? There?s no rulebook, but I have a chapter of ethical and legal considerations in which I delve into the writer?s responsibility to the subject.?

Talk about the marketplace, especially for travel writers.

I think there?s always a market for a well-written travel book.? I can?t say for certain because I?m not a marketer, but it seems to be a pretty strong niche.?

Quite a few publishers now print disclaimers on an opening page of a memoir that names have been changed.??Which makes me wonder: Should a memoir writer allow his or her subjects to vet the manuscript at any point along the way?

That?s a good question.? It really depends on what you?re writing about.? When I was writing Do-Over, one of the people I wanted to write about said that she didn?t want to be ?an incidental character in your adventure.?? I liked that. It made me think about my relationship to the people I wrote about.? In her case, I showed her what I had written and changed her name, but others didn?t ask that of me. I changed the names of anyone under 18 except for my daughters, but I also gave them the chance to read the manuscript before I sent it to the publisher.?

??????????? When I wrote Invented Eden, I didn?t have the luxury of changing names, and I needed to make sure that nothing I wrote was actionable.

??????????? Traditionally, journalists have resisted showing what they write to their subjects, but in some instances, that?s something worth reconsidering.?

Some have said the memoir genre is a crowded one. What do you think?

Every genre is crowded.? Thousands of novels are published every year.? Thousands of books of poetry.? There are plenty of good books and plenty of bad books of every kind.?

What's your opinion of the mountains of self-published books, and do you

think they help or hinder the publication and marketing of traditionally

published books?

I know of a couple of people who?ve self-published books successfully, but they are definitely the exception.? While I don?t think it carries the stigma it used to, there are so many exciting independent presses out there now.? Not only is it easier to self-publish, but it?s also easier to publish other people, to start your own press.? One start-up, Engine Books in Indianapolis, has published a short story collection and a novel by two friends of mine, both fabulous writers who might be relegated to the mid-list with traditional houses.? But they?ve been doing well?this publisher publishes two to three titles a year, and my friend?s short story collection was just reviewed in O: the Oprah Magazine.? So why self-publish when there are so many good independent publishers out there?

Many of your books have been published by smaller presses. In other words, not by the Big Six. Talk about the value of smaller presses.

I wouldn?t necessarily call them smaller presses. Three of my books have been published by Graywolf, which isn?t exactly a small press anymore, but a giant of independent presses.

??????????? In any case, I?d prefer to call these presses ?Independent? rather than ?small.?? My father was the founder and publisher of just such an Independent Press, the somewhat legendary Noonday, which in the fifties was what Graywolf is today.

??????????? One of the biggest strengths of independent presses is that they don?t necessarily have to be as slavish to the bottom line as some of the larger presses are.? They can take chances, especially those that are nonprofit, such as Graywolf.

??????????? A case in point is Noonday.? When my father was first starting it up in the 1950s he and my mother attended a cocktail party given by the owner of The New Yorker Theater, Dan Talbot.? In the middle of the party, Talbot shushed everyone and said he wanted to read this great new story that had just appeared in the latest issue of The Partisan Review, an important literary magazine of its day.? Can you imagine anyone at a party doing such a thing now?? The story in question was a translation by Saul Bellow of a then-unknown Yiddish writer, Isaac Bashevis Singer.

??????????? Talbot read the story and my father asked if Talbot knew Singer.? He did and a meeting was arranged.? It turned out that Signer was upset by the way that Alfred Knopf had edited his first novel, The Family Moskat.? According to Singer, Knopf had butchered the book, and so Singer was amenable to working with my father.? Singer and my father were a great match, and my father became Singer?s translator, editor, and trusted friend.

??????????? Independent publishers love literature.? That can?t always be said for the big corporate houses.? Matthew Arnold said that ?journalism is literature in a hurry,? but now literature is literature in a hurry.? What I mean is that that nurturing spirit is largely absent from large houses: If you don?t succeed right away, you?re out.? But independent publishers still nurture their writers for the most part.?

Let?s talk about teaching. How do you teach nonfiction writing?

That?s a difficult question to answer?but briefly I do it through a combination of manuscript critique, peer critique, and individual conferencing.? But it really depends on the venue: a traditional university, a weekend workshop, or a low-residency distance-learning model.? They all demand different approaches.? In general, I give students models in the genre in which they?re working, I sometimes use writing exercises (again, depending on the level), and I try to see what the writer is attempting to do and help him or her achieve that goal.? I?m working with so many different kinds of nonfiction writers that I have to tailor my approach somewhat to each writer.

How does teaching help or hinder your own work?

Sometimes it helps, sometimes it hinders.? It depends on the time of year and the students with whom I?m dealing.? I work with some mightily talented students and while I don?t usually find myself directly inspired by their work, I do get a vicarious pleasure in helping them turn their ideas into essays and books.?

HBT: Teixeira out 1-2 weeks with calf strain

11:27 PM: According to Sweeny Murti of WFAN, Mark Teixeira said he was diagnosed with a Grade 1 calf strain and thinks he could be out anywhere from one to two weeks. Of course, it?s possible he could miss more time.

11:10 PM: No word yet on the MRI results, but Yankees manager Joe Girardi told Jack Curry of YES Network that Teixeira is unlikely to play in at least the next five games.

9:10 PM ET: Mark Teixeira?s left wrist has been a nagging issue for the past month, but now he has another injury to deal with.

Bryan Hoch of MLB.com reports that Teixeira exited tonight?s game against the Blue Jays after four innings with a left calf strain and was sent to New York-Presbyterian Hospital to undergo an MRI.

Teixeira appeared to tweak something during his at-bat in the fourth inning. He finished the at-bat and eventually drew a walk, but limped around the bases when he scored on an RBI single by Russell Martin. Eric Chavez replaced him at first base to begin the fifth inning while Jayson Nix entered the game to play third.

Teixeira is hitting .255/.355/.478 with 23 home runs, 81 RBI and an .813 OPS in 119 games this season. The Yankees could shuffle through a number of options at first base if he requires an extended absence, including Nick Swisher, Eric Chavez, Casey McGehee and the newly-acquired Steve Pearce.

Green Blog: Hunting for Debris and Answers in Alaska

Throughout months of measuring trees, I carried the hope of finding a glass float on the outer coast of Southeast Alaska. Green and blue and made of hand-blown glass, they are remnants of the deep sea fishing industry, used long before plastic and other materials kept nets afloat. Most that remain today originated in Japan. Since I last wrote for the Green Blog, we?ve visited 24 sites in dead, dying and stressed yellow cedar forests in the West Chichagof-Yakobi Wilderness over the course of a three-week expedition.

In the rain, wind and impenetrable fog that cloaked our final push for data collection, twice my heart jumped at the sight of the sacred glass on rocky shores. I reached down with my neoprene-covered hands, only to claim a large light bulb on both occasions. These too, washed up from Japan alongside the Styrofoam and bottles. Predictions from the National Oceanic Atmospheric Administration say that by 2013, 1.5 billion tons of debris from the tsunami could reach the coast of Alaska down to California. Out there on the outer coast, we may have stumbled upon the earliest signs.

I?m here in the Alexander Archipelago to study the forests ? what happens after the yellow cedars die ? but this project continually uncovers the unexpected. I?m learning about a history of wilderness and changes occurring on protected lands. I?m learning about ocean currents. I?m learning about gold mining and fishing regulation. I?m learning how to let tides and swell dictate my course and how to be fully present.

I set a goal for data collection, but the daily work and life in harsh conditions demands constant attention, persistent energy and flexibility in planning.

Alongside giant Sitka spruce and regenerating hemlock trees, spouting humpback whales and grizzly bears, I?ve been alone for weeks in some of the nation?s most remote wilderness with only the company of my team members: Tom?s Ward, Gregg Treinish and Corey Radis. We note signs of human activity like visitors to a far-off planet: the scarce fishing boats, float planes and jets above, the trash where land meets sea. But mostly, solitude pervades. When out there on the coast, I focus on staying safe, recording measurements of tree heights and sapwood from cores we collect, and moving forward.

It?s difficult to imagine anything beyond the perpetual shades of gray. We haul our kayaks up the jagged beach before jumping into the wall of forest each day toward a successful mission: sites completed, temperature devices reclaimed, trees measured and cores collected across miles of surveyed coastline.

We swapped the healthy forests and chilly summer air in Glacier Bay National Park for the steady rain and dying yellow cedars farther south in the Tongass National Forest. The Island of Chichagof in the Tongass is part of what southeast Alaskans call the ABC islands ? Admiralty, Baranof, Chichagof ? the three large islands that make up the northern portion of the archipelago, known for their desolation and wildlife.

Chichagof has the highest density of bears per square mile of any place documented on earth ? with more bears than there are people dotting the few, rural communities.

Like many parts of Alaska, Chichagof carries a mining history. Not far from the forests in Klag Bay is a low-priority Superfund site; decaying boats and rusted barrels remain from a gold mine closed in 1942. In 1980, the western, coastal region of the island was federally designated as wilderness, offering the highest level of national preservation. It is area many Alaskans consider the gem of the wild archipelago.

My research on the ecological responses to yellow cedar decline led me here. I needed to work in a place relatively undeveloped, in forests untouched by the logging that has occurred throughout the Tongass.

When the Wilderness Act passed in 1964, it set aside 9.8 million acres of federal land for the preservation of natural conditions and for the use and enjoyment of those lands as wilderness. Wilderness areas today cover roughly 110 million acres of land managed by the National Park Service, the Forest Service, the Fish and Wildlife Service and the Bureau of Land Management.

Yet wilderness is not easily defined. The historian Roderick Nash wrote, ?civilization created wilderness.? Others, like William Cronon, also argued that wilderness was a human invention based on a romantic ideology. Wilderness ? with a capital ?W,? according to the Wilderness Act ? offers the values of ?solitude,? and ?primitive, unconfined recreation.?

Measuring impacts to the experience of solitude or balancing the protection of natural conditions with various uses is no easy task.

One day, we leave our base camp at Waterfall Cove to visit four sites, and upon returning, find a fishing boat has capsized, spilling nearly 1,000 gallons of diesel fuel and petroleum, forcing managers to close the fishery in Slocum Arm. When the fog ceiling lifts, we count float planes buzzing in the distance. We reach the open ocean in our kayaks and note more debris.

Yet deep in the forest, signs of grizzly and Sitka black-tailed deer are the only paths we can follow amidst all the bushwhacking.

Perhaps one day, the systematic discipline of science will lead me to a few solid answers about what happens after the yellow cedar die, and on to a lot more questions relevant to other people and places on this planet.

Swedish pioneer launches networking site for top 1% users

Swedish social media entrepreneur Erik Wachtmeister on Monday announced the launch of a new social networking site targeting the "top one percent" of Internet users, called Best of All Worlds.

The site, which helps users find events and contacts, counted 20,000 members even before its launch.

Wachtmeister, 57, created one of the world's first social networking sites, aSmallWorld, in 2004, the same year as Facebook. He left the company in 2008.

Best of All Worlds is targeting the same audience, he said.

"The top one percent of the online audience, people who are leaders in their field, investment bankers, PR people, media, fashion, government... It's not about jet-set or rich people, but sophisticated people who have good taste," he said.

"It's more three million people than a billion," he added, a reference to Facebook's announcement in July that it had more than 950 million subscribers.

Best of All Worlds helps users get in touch with each other to find what's going on where they are, which Wachtmeister said was an improvement on Facebook's service which "looks backwards (and) gives you unstructured information that shows what your friends have done."

Subscription to Best of All Worlds is by invitation only. Wachtmeister did not specify how or when the company would turn a profit.

Tropical Storm Isaac drenches Haiti, swipes Cuba

PORT-AU-PRINCE (Reuters) - Tropical Storm Isaac dumped torrential rains on Haiti and flattened tent camps housing survivors of a devastating earthquake, then began an assault on eastern Cuba on Saturday.

Isaac killed at least two people in Haiti and was expected to strengthen into a hurricane before hitting the Florida Keys on Sunday and crossing into the Gulf of Mexico.

Fueled by warm Gulf waters, it was forecast to strengthen into a Category 2 hurricane with 100-mph (160-kph) winds and hit the U.S. coast somewhere between the Florida Panhandle and New Orleans at midweek.

Isaac's march toward the Gulf comes as U.S. Republicans prepare to gather in Tampa, on Florida's central Gulf Coast, for Monday's start of their national convention ahead of the November presidential election. The convention is expected to proceed as planned.

Energy operators in the Gulf of Mexico were shutting down offshore oil and gas rigs ahead of Isaac. The storm could spur short-term shut-downs of 43 percent of U.S. offshore oil capacity and 38 percent of its natural gas output, according to forecasters at Weather Insight, an arm of Thomson Reuters.

Isaac's rain and winds lashed Haiti's southern coast on Saturday, flooding parts of the capital Port-au-Prince and ripping through flimsy resettlement camps that house more than 350,000 survivors of the 2010 earthquake.

A 10-year-old girl was killed in Port-au-Prince when a wall fell on her and a woman in the southern coastal city of Jacmel was crushed to death when a tree fell on her house, government officials said.

At a tent camp in the seaside slum of Cite Soleil, corrugated plastic shacks were broken apart and water gushed in.

"We had never seen anything like this. Everyone fled to the church, but I didn't want to leave my home. All my things are wet," said Edeline Trevil, 47, who survived with her cat.

"I'm cold! I've been wet since last night," the shoeless woman added.

The storm caused power outages and flooding and blew off roofs as it moved across the hilly and severely deforested Caribbean country. Winds had died down by Saturday afternoon but forecasters said rains would continue in Haiti.

EMERGENCY IN FLORIDA

Damage had so far been less than feared, said George Ngwa, Haiti spokesman for the United Nations Office of Coordination of Humanitarian Affairs.

"We don't have any serious reports of major flooding or mudslides so far," he said.

By mid afternoon on Saturday, Isaac's center was over eastern Cuba, 40 miles north of Guantanamo Bay, the U.S. National Hurricane Center said.

The storm had top winds of 60 mph and would become a hurricane if those swirling winds reach 74 mph. A hurricane warning was in effect for the Florida Keys and the southwest coast of Florida.

In Haiti, the United Nations mission said it was prepared to distribute food and emergency supplies for more than 300,000 people, and 10,000 U.N. troops, including several engineering units, stood ready to clear roads for emergency response teams.

The government and aid groups tried to evacuate thousands of tent camp dwellers on Friday but many Haitians chose to remain in their flimsy, makeshift homes, apparently fearing they would be robbed, said Bradley Mellicker, head of disaster management for the International Organization for Migration (IOM).

Flooding and mudslides were still a threat in Haiti, where many people scrape by on less than $1 per day in the poorest country in the Americas. Flooding could also reignite a cholera epidemic, which has killed more than 7,500 people in Haiti since the disease first appeared in October 2010, aid workers said.

In the Dominican Republic, Isaac felled power and phone lines and left at least a dozen towns cut off by flood waters. The most severe damage was reported along the south coast, including the capital Santo Domingo, where more than half the city was without power.

Rising winds and waves whipped eastern Cuba on Saturday afternoon, prompting government alerts for 11 provinces. In Cuba's easternmost city of Baracoa, water crashed over the seawall and ran ankle-deep through the nearby streets.

More than 1,000 people were evacuated to the homes of friends and family ahead of possible flooding, Cuban TV said.

"We fear the sea and the flooding a lot more than the rain and wind. It rains frequently here all year long, but when the sea comes in everything floods," said Baracoa housewife Yamila Sanchez.

The storm was expected to pass directly over the town of Moa, the center of the Cuban nickel mining industry, but officials said they expected no major problems and had enough ore on hand to continue processing for up to five days. Cuba is one of the ten largest nickel producers in the world.

In Florida, Governor Rick Scott declared a state of emergency, an administrative step aimed at streamlining disaster preparations.

Emergency managers urged tourists to leave the Florida Keys if they could do so safely on Saturday. A single road links the chain of low-lying islands to the Florida Peninsula and the Key West airport was expected to halt flights on Saturday evening.

At Cape Canaveral on Florida's east coast, squalls from the storm delayed until next week the launch of a pair of NASA satellites to study Earth's radiation belts.

Isaac has drawn especially close scrutiny because of the Republican Party's convention, a four-day meeting during which former Massachusetts Governor Mitt Romney will receive the party's presidential nomination.

Party officials insist the convention will go ahead, even if they have to alter the schedule. But Hurricane Center meteorologist Rick Danielson said Tampa could be hit by coastal flooding and driving winds or rain.

"There is still a full range of possible impacts on Tampa at this point," he said.
